Grievance & Appeals Policy

Divon Destiny respects the rights of Medicaid participants to voice concerns, file grievances, and request appeals without fear of retaliation.


Grievances A grievance is an expression of dissatisfaction regarding services, staff, or operations. Grievances may be submitted verbally or in writing.


Appeals Participants may request an appeal regarding service-related decisions in accordance with Virginia Medicaid guidelines.


How to File Grievances or appeals may be submitted:

  • By phone

  • By email

  • In writing

All concerns are acknowledged promptly and addressed within required timeframes.
