Who We Are

 

Divon Destiny is a Virginia‑based Service Facilitation agency committed to promoting independence, while dignity, and choice for Medicaid waiver participants. We deliver participant‑centered services supporting adherence to state and federal Medicaid requirements.

 


Our Role

We assist participants and their chosen representatives with service planning, attendant management, and ongoing support. Our role is to help individuals successfully self‑direct their services with clarity and confidence.

 


Credentials & Compliance

  • Enrolled with Virginia DMAS

  • Required Service Facilitator training completed

  • HIPAA‑compliant systems and safeguards

  • NPI/Provider ID available upon request

Consumer-Directed vs Agency Directed

 

Consumer Directed contrasts with Agency Directed in many ways. With Agency Directed, the providers makes decisions on behalf of the consumers, which sometimes lead to a one-size-fits-all approach that doesn't fully address individual needs or preferences. Agency Directed does not allow you to choose your own attendant, one is attached/assigned to you. Overall, the Consumer Directed Model fosters empowerment and self-determination for those receiving services.

 


Utilizing the Consumer Directed Model

 

An EOR must be established. The EOR is responsible for hiring, training, supervising and termination of the attendant who provides the direct support to the waivered individual. A Services Facilitator, like Divon Destiny, must be chosen to begin services. You must agree to routine in-home reviews of your services with your SF held on a monthly or bi-monthly basics according to your MCO schedule.

Takahya Everett-Gatlin 

Takahya Everett-Gatlin is an experienced special education professional, entrepreneur, and disability services leader with more than 20 years of experience supporting individuals with intellectual and developmental disabilities. Based in Northern Virginia, she has built her career around helping individuals and families navigate complex service systems while promoting independence, dignity, and meaningful community inclusion.

 

She is the founder of DIVON Destiny, an organization focuse on supporting individuals and families as they navigate disability service systems and access person-centered resources. Through her work, Takahya emphasizes practical life skills, structured support systems, and real-world strategies that help individuals maintain stability and increase independence. Her approach combines hands-on experience, regulatory knowledge, and a strong commitment to quality service delivery.

 

In addition to leading DIVON Destiny, Takahya is expanding services into consumer-directed service facilitation and companion care, and is in the process of launching a non-medical home care agency to broaden support options for individuals and families who need reliable in-home assistance.

 

Known for her direct leadership style and strong understanding of Medicaid waiver systems, Takahya also develops training materials, operational systems, and compliance tools designed to help providers remain organized, accountable, and audit-ready while delivering high-quality support.

 

Her mission is clear: build services and systems that empower people to live independently, strengthen families, and improve the quality of support available in the community.
