CCC Plus Participation Statement

Divon Destiny provides Service Facilitation to individuals enrolled in the Virginia Medicaid CCC Plus Waiver. Services are delivered in accordance with DMAS regulations and applicable Managed Care Organization (MCO) requirements.

Participation in CCC Plus does not limit a member’s freedom of choice of providers. Divon Destiny does not guarantee eligibility, authorization, or payment for services, which are determined by Medicaid and the member’s health plan.

We coordinate professionally with all CCC Plus managed care plans, including but not limited to Anthem, Aetna, Molina, and UnitedHealthcare, while remaining plan-neutral.
