What Is a Service Facilitator?

A Service Facilitator supports individuals enrolled in Virginia Medicaid waiver programs who choose consumer‑directed services.

 

Divon Destiny guides participants and employers of record through planning, hiring, training, and ongoing management of attendants—while supporting compliance with DMAS requirements.

Divon Destiny provides Non-Medical Support Services and Service Facilitation to individuals and families receiving waiver-funded services. Services are person-centered, community-based, and delivered in alignment with DBHDS requirements and the Individual Support Plan (ISP).
